? - Query or suspected. The question mark symbol is in general use in medicine and is used when there is a suspicion of an injury or condition...or anything for that matter. Technically, the ? is used when you cannot be sure - a closed fracture is a good example. Without an x-ray there is no way we can define the injury, even if we are sure. An open fracture, where the bone is sticking out would not require a ? - it's an obvious fracture.
